ABSTRACT

Introducción: La sutura mecánica es una opción para el cierre de la faringorrafia en laringectomía total por cáncer de laringe. Objetivo: Comparar el uso de sutura mecánica lineal con sutura manual de la faringe durante la laringectomía total por cáncer de laringe. Material y Método: Se realizó un estudio de tipo experimental prospectivo. Se compararon dos grupos pacientes: Los pacientes con sutura mecánica desde 2018 a marzo de 2020 y los pacientes con sutura manual previa a enero de 2018 en el Servicio de Otorrinolaringología del Hospital Barros Luco. Se analizaron fístula faringo-cutánea posoperatoria (FFC), tiempo operatorio, estadía hospitalaria y costo. Resultados: El estudio incluyó a 14 pacientes, cada grupo con n = 7. El grupo con sutura mecánica presento 0% de FFC y el grupo sutura manual 28%. El grupo con sutura mecánica reinicio alimentación a los 7 días y el grupo con sutura manual en promedio a los 11,5 (7-23) días. (p = 0,0023). El tiempo promedio de cirugía para el grupo experimental es de 288 ± 37,4 minutos y con sutura manual 311 ± 32,4 minutos. (p = 0,0176). El promedio de hospitalización para el grupo experimental fue de 11 ± 2,6 días (9 a 16), y para el grupo control fue de 21 ± 14 días (10 a 49) (p < 0,0001). Conclusión: La sutura mecánica es un procedimiento fácil de usar y seguro. Existiría un beneficio en el uso de sutura mecánica para el cierre faríngeo al compararlo con la sutura manual al disminuir el número de FFC, el tiempo operatorio y los días de hospitalización.


Introduction: Mechanical suture is an option for the closure of the pharyngorrhaphy in total laryngectomy due to laryngeal cancer. Aim: To compare the use of linear mechanical suture with manual pharyngeal suture during total laryngectomy for laryngeal cancer. Material and Method: A prospective experimental study was carried out. Two patient groups were compared: patients with mechanical suture from 2018 to March 2020 and patients with manual suture prior to January 2018 at the otorhinolaryngology service of the Barros Luco Hospital. Postoperative pharyngocutaneous fistula (FFC), operative time, hospital stay and cost were analyzed. Results: The study included 14 patients, each group with n = 7. The group with mechanical suture presented 0% of FFC and the group with manual suture 28%. The group with mechanical suture restarted feeding at 7 days and the group with manual suture on average at 11.5 (7-23) days (p = 0.0023). The average surgery time for the experimental group was 288 ± 37.4 minutes, while for the group with manual suture was 311 ± 32.4 minutes (p = 0.0176). The mean hospitalization for the experimental group was 11 ± 2.6 days (9 to 16), and for the control group it was 21 ± 14 days (10 to 49) (p < 0.0001). Conclusion: The mechanical suture is an easy to use and safe procedure. There would be a benefit in the use of mechanical suture for pharyngeal closure when buying it with manual suture by reducing the number of FFCs, operative time and days of hospitalization.

ABSTRACT

El onfalocele o exónfalos se definen como un defecto congénito de la pared abdominal, que consiste en la herniación de las vísceras abdominales a través del anillo umbilical. Esta entidad rara vez se asocia a la comunicación del divertículo de Meckel con el saco del onfalocele. Teniendo en cuenta la escasa prevalencia de dicha entidad, compartimos el reporte de caso de un paciente recién nacido, con diagnóstico de onfalocele menor, en quien se sospechaba ruptura del saco, sin embargo, de manera intraoperatoria se encontró que la aparente ruptura del saco, correspondía a la comunicación con un divertículo de Meckel. El caso además se asoció con hallazgos ecocardiográficos de tetralogía de Fallot


Omphalocele or exomphalos are defined as a congenital defect of the abdominal wall, which consists of the herniation of the abdominal viscera through the umbilical ring. This entity is rarely associated with the communication of Meckel's diverticulum with the omphalocele sac. Considering the low prevalence of this entity, we report the case of a newborn with a diagnosis of minor omphalocele, in whom rupture of the sac was suspected; however, intraoperatively it was found that the apparent rupture of the sac corresponded to a communication with a Meckel's diverticulum. The case was also associated with echocardiographic findings of tetralogy of Fallot

ABSTRACT

Introducción: La hemorroidopexia con sutura mecánica, es una técnica eficaz para los prolapsos hemorroidales grado III y IV. Objetivo: Describir nuestra experiencia con dicho método. Material y métodos: Se realizó un análisis descriptivo, retrospectivo sobre una base de datos cargada en forma prospectiva. Se analizaron pacientes con hemorroides grado III y IV, en quienes se realizó hemorroidopexia con PPH- 03, desde enero del 2010 hasta diciembre de 2017. Se evaluaron las variables demográficas, las complicaciones y el grado de satisfacción. Se realizó seguimiento postoperatorio semanal, a los 15 días, al mes, a los 6 meses y finalmente un control telefónico. Resultados: Se estudiaron 452 pacientes. La edad media fue de 46 años (rango: 20-75), siendo el 63% (n=291) del sexo masculino. El 84% (n=387) presentaban hemorroides grado III. El índice de complicación fue del 25% (n=115) durante el periodo de seguimiento, en el postoperatorio inmediato, 60 (52%) pacientes presentaron un evento considerado como complicación, y 55 (48%) pacientes presentaron complicaciones mediatas y tardías. Las complicaciones más frecuentes fueron: dolor en 37 (8%) de los pacientes, seguida por proctorragía en 18 casos (4%) y trombosis hemorroidal externa en 15 pacientes (3,3%). (AU)


Background: Hemorrhoidopexy with mechanical suture is an effective technique for hemorrhoidal prolapses grade III and IV. Objective: Describe our experience with this method. Material and methods: A descriptive, retrospective analysis was performed on a prospectively loaded database. Patients with grade III and IV hemorrhoids were analyzed, in whom hemorrhoidopexy was performed with PPH-03, from January 2010 to December 2017. Demographic variables, complications and degree of satisfaction were evaluated. Weekly postoperative follow-up was performed at 15 days, at month, at 6 months and finally a telephone control. Results: 452 patients were studied. The average age was 46 years (range,: 20 - 75), being 63% (n = 291) of the male sex. 84% (n = 387) had grade III hemorrhoids. The complication rate was 25% (n = 115) during the follow-up period, in the immediate postoperative period, 60 (52%). The most frequent complications were: pain in 37 (8%) patients, follow-up by proctorrhagia in 18 cases (4%) and hemorrhoidal thrombosis external in 15 patients (3.3%). (AU)

ABSTRACT

Introducción: El procedimiento ideal para el tratamiento de la Enfermedad Hemorroidal debe aliviar los síntomas, conservar la funcionalidad anorrectal y minimizar las complicaciones. En los Grados III y IV estos objetivos representan un verdadero desafío. Objetivo: Comunicar la experiencia de un equipo quirúrgico con el procedimiento de Hemorroidopexia con sutura mecánica y revisar la evidencia nacional e internacional. Diseño: Análisis retrospectivo sobre una base de datos prospectiva. Material y método: Entre junio de 2007 y diciembre de 2016, fueron intervenidos 135 pacientes con Enfermedad Hemorroidal sintomática Grados III y IV. Se analizan las variables demográficas, indicaciones, técnica, morbilidad a corto y largo plazo. Resultados: 59% de los operados fueron mujeres. La edad promedio con todos los procedimientos fue de 51 años. El 52% de los pacientes presentaban Hemorroides Grado III y el 47% Hemorroides Grado IV. La Hemorroidopexia se realizó como Cirugía ambulatoria en el 17% de los casos. El índice global de complicaciones fue de 11,11%, que agrupamos según Clasificación de Dindo-Clavien. No se registraron complicaciones graves ni mortalidad. Conclusiones: La Hemorroidopexia con sutura mecánica es un procedimiento seguro y efectivo para el tratamiento de los grados avanzados de Enfermedad Hemorroidal. Puede llevarse a cabo en forma ambulatoria. Son esenciales la correcta indicación y capacidad para el manejo de probables complicaciones. (AU)


Background: The best surgical procedure for the Hemorrhoidal Disease must relieve the symptoms, retain anorrectal functionality and minimize complications. At Grades III and IV those aims are a real challege. Objective: To comunicate the one surgical team experience whit stapled Hemhoirrodopexy and to analyse the national and international evidence. Design: Análisis retrospectivo sobre una base de datos prospectiva. Methods: From June 2007 to December 2016 we operated 135 patients to have Hemorrhoidal Disease Grade III and IV. We described demographics dates, indications, and surgical procedure, short and long morbidity. Results: 59% of the patientes was female. In all procedures, de mean age was 50. 52% of the patients was Hemhorroid Disease Grade III and 47% was Grade IV. The Stapled Hemhoirrodopexy was Ambulatory in 17%. The global morbidity was 11,11%. No, have mortality with the surgical procedure. Conclusions: The Stapled Hemohoirodopexy is a safe and effective surgical procedure to treatment of symptomatic Grade III and IV of Hemorrhoidal Disease. It can be carried out as ambulatory surgery. Are clues the right indication and the capacity to resolve complications. (AU)

ABSTRACT

Background: The occurrence of the pharyngoesophageal, or Zenker diverticulum is not frequent in the national scenario, and the technique of the diverticulectomy with cricomyotomy in medium and great dimension diverticula is still the most indicated. Because the resection of the diverticulum requires the suture of the pharynx, dehiscence can occur, thereafter delaying swallowing. Hence, the idea is to accomplish this surgical procedure, comparing the manual and mechanical suture, in order to evaluate the real benefit of the mechanical technique. Aim: To evaluate the results of the pharyngoesophageal diverticulectomy with cricomyotomy using manual and mechanical suture with regard to local and systemic complications. Method: Fifty-seven patients with pharyngoesophageal diverticula diagnosed through high digestive endoscopy and pharyngeal esophagogram were studied. The applied surgical technique was diverticulectomy with myotomy of the cricopharyngeal muscle, done in 24 patients (42.2%) the mechanical suture (group A) with the mechanical linear suture device and in 33 (57.8%) a manual closure of the pharynx (group B). Results: In the postoperative period, one patient of group A (4.1%) presented fistula caused by dehiscence of the pharyngeal suture, and three of group B (15.1%) presented the same complication, with a good outcome using a conservative treatment. In the same group, three patients (9.0%) presented stenosis of the suture of the pharynx, with good outcome and with endoscopic dilatations, and no patient from group A presented such complication. Lung infection was present in five patients, being two (8.3%) of group A and three (9.0%) on B, having good outcomes after specific treatment. In the late review, done with 43 patients (94.4%) of group A and 22 (88.0%) on B, the patients declared to be pleased with the surgical procedure, because they were able to regain normal swallowing. Conclusion: The diverticulectomy with myotomy and pharyngeal closure using mechanical suture was proven appropriate, for having restored regular swallowing in most of the patients, and the mechanical closure of the pharynx proved to be more effective in comparison to the manual one, because it provided a lower index of local post-surgical complications.


Racional: A ocorrência do divertículo faringoesofágico, ou de Zenker, é pouco frequente no cenário nacional, sendo que a técnica da diverticulectomia com cricomiotomia em divertículos de média e grandes dimensões ainda é a mais indicada. Devido à ressecção do divertículo necessitar de sutura da faringe ocorre possibilidade de deiscência, o que retarda o retorno da deglutição. Daí a ideia de realizar este procedimento cirúrgico, comparando a sutura manual com a mecânica, para avaliar o real benefício da técnica mecânica. Objetivo: Avaliar os resultados da diverticulectomia faringoesofágica com cricomiotomia utilizando à sutura manual e mecânica em relação às complicações locais e sistêmicas. Métodos: Foram estudados 57 pacientes com divertículos faringoesofágicos diagnosticados através da endoscopia digestiva alta e faringoesofagograma. A técnica cirúrgica empreendida foi a diverticulectomia com miotomia do músculo cricofaríngeo, sendo a sutura mecânica realizada em 24 pacientes (42,2%, grupo A) com o aparelho linear e em 33 (57,8%, grupo B) a manual para o fechamento da faringe. Resultados: Na avaliação do pós-operatório precoce, um paciente do grupo A (4,1%) apresentou fístula consequente à deiscência da sutura da faringe e três do grupo B (15,1%) ambos com boa evolução com tratamento conservador. Neste mesmo grupo, três pacientes (9,0%) apresentaram estenose da sutura da faringe, com boa evolução com dilatações endoscópicas sendo que nenhum do grupo A apresentou esta complicação. A infecção pulmonar esteve presente em cinco pacientes, dois (8,3%) do grupo A e três (9,0%) do grupo B, com boa evolução com tratamento específico. Na avaliação tardia, realizada em 43 pacientes, 17(94,4%) do grupo A e 22 (88,0%) do grupo B, os pacientes referiram estarem satisfeitos com o procedimento cirúrgico, pois conseguiram resgatar a deglutição normal. Objetivo: El objetivo del presente trabajo es dar a conocer la experiencia de nuestro grupo de Cirugía Plástica, en sutura mecánica microvascular utilizando el dispositivo Coupler® (Synovis Corp, Birmingham, AL) para las anastomosis venosas realizadas en pacientes sometidos microcirugía reconstructiva. Material y Método: Revisión retrospectiva de los últimos 48 colgajos libres operados entre marzo de 2009 y febrero de 2013. Todas las anastomosis microquirúrgicas venosas fueron realizadas con el dispositivo Coupler® y las anastomosis arteriales con sutura separada o continua de nylon 9-0 ó 10-0. Los datos recolectados fueron: datos personales y médicos del paciente, localización del defecto, colgajo utilizado, diámetro de los vasos, anastomosis arterial, Coupler® utilizado, tiempo de anastomosis, complicaciones postoperatorias, trombosis arterial y trombosis venosa. Resultados: Fueron realizados 48 colgajos libres para reconstrucción de extremidad inferior (n = 25, 52,1 por ciento); cabeza-cuello (n = 15, 31 por ciento); mama (n = 5, 10,4 por ciento) y extremidad superior (n = 3, 6,3 por ciento). Los colgajos utilizados fueron: Anterolateral de muslo o ALT (n = 25, 52,1 por ciento), Radial (n = 10, 20,8 por ciento), Perforante de Arteria Epigástrica Inferior o DIEP (n = 5, 10,4 por ciento), Dorsal Ancho (n = 2, 4,2 por ciento), Recto Abdominal (n = 2, 4,2 por ciento), Fíbula (n = 2, 4,2 por ciento), Escápula (n = 1, 2,1), y Gracilis (n = 1, 2,1 por ciento). El diámetro promedio del dispositivo Coupler® utilizado en el total de los casos fue de 2,51 +/- 0,46 mm. El tiempo promedio para la anastomosis venosa fue de 13,5 +/- 7,3 min. Un total de 4 colgajos se perdieron (8,3 por ciento), ninguno a causa de falla venosa. Conclusiones: El Coupler® venoso es un elemento seguro, efectivo y rápido para la realización de las anastomosis microquirúrgicas, es fácil de aprender y su implementación debiese ser considerada en todo centro donde se realice microcirugía.


Aim: The aim of the present paper is to show the experience of the Plastic Surgery Division at the University of Chile Clinical Hospital, with the Coupler® anastomotic device (Synovis Corp, Birmingham, AL) for venous microanastomoses in patients undergoing reconstructive microsurgery. Material and Methods: A retrospective review of 48 consecutive patients with free flaps between March 2009 and in February 2013. All microsurgical venous anastomoses were performed with the Coupler® device and the arterial anastomoses with interrupted or continuous 9-0 or 10-0 nylon sutures. The collected data were: personal and medical patient information, location of the defect, flap used, vessels diameter, arterial anastomoses, Coupler® used, time of anastomosis, postoperative complications, arterial and venous thrombosis. Results: A total of 48 free flaps were performed for reconstruction of lower extremity (n = 25, 52.1%); head-neck (n = 15, 31%); breast (n = 5, 10.4%) and upper extremity (n = 3, 6.3%). The flaps used were: Anterolateral thigh or ALT (n = 25, 52.1%), Radial (n = 10, 20.8%), Deep Inferior Epigastric Artery Perforator or DIEP (n = 5, 10.4%), Latisimus Dorsi (n = 2, 4.2%), Rectus Abdominus (n = 2, 4.2%), Fibula (n = 2, 4.2%), Scapula (n = 1, 2.1%), and Gracilis (n = 1, 2.1%). The Coupler device average diameter used was 2.51 ± 0.46 mm. The average time for the venous anastomosis was 13.5 ± 7.3 minutes. A total of 4 flaps were lost (8.3%), none due to venous thrombosis. Conclusions: The Coupler® System for venous anastomoses is a safe, effective and quick method for microsurgical anastomoses, it is easy to learn and its implementation should be considered in every microvascular surgery center.

ABSTRACT

INTRODUCCION: entre las técnicas descriptas para corregir defectos del piso pelviano, el uso de suturas mecánicas circulares se plantea como una opción con fundamentos anatómicos y funcionales, con bajos índices de complicaciones. OBJETIVO: analizar las indicaciones y técnicas utilizando suturas mecánicas circulares por vía Endoanal, su factibilidad y resultados inmediatos. DISEÑO: estudio prospectivo, consecutivo, no randomizado. MATERIAL Y METODO: entre Junio de 2007 y Diciembre de 2010 se utilizaron los dispositivos PPH 03 y PPH 01 en 62 pacientes. Se analizan: indicaciones, técnica, tiempo operatorio, morbilidad, internación y resultados funcionales inmediatos. RESULTADOS: con el procedimiento PPH fueron intervenidos 40 pacientes. Con la técnica STARR fueron operadas 22 mujeres. Para el procedimiento PPH el tiempo operatorio promedio fue de 30 minutos y la morbilidad 15%. Para STARR el tiempo operatorio promedio fue de 40 minutos y la morbilidad 4,5%. Con la técnica PPH se obtuvo 100% de corrección de prolapso. Con STARR se observó disminución del ODS Score en todos los casos. Internación promedio: 1 día. CONCLUSIONES: el uso de los dispositivos de sutura mecánica circular por vía endoanal es factible y seguro. Utilizados por coloproctólogos con experiencia ofrecen una alternativa válida en los pacientes que cumplan los requisitos para indicarlos.


BACKGROUND: several techniques have been described for the treatment of pelvic floor diseases. The circular stapler devices are advocated as a safety and effectiveness option, with anatomical and physiological basis. OBJECTIVE: The aim of this study was to demonstrate the feasibility, effectiveness and reliability of the circular staplers devices for endoanal techniques. DESIGN: non-randomized, prospective study. MATERIAL AND METHODS: since June 2007 up to December 2010 we have applied the PPH 03 and PPH 01 devices to 60 patients. We analyzed indications, surgical techniques mean operative time, complications, mean length of stay and immediate functional results. RESULTS: we have applied the PPH procedure to 40 patients and the STARR procedure to 22 female patients. To PPH procedure the mean duration of the operation was 30 minutes and we registered a rate of complications to 15%. To STARR procedure the mean operative time was 40 minutes and the morbidity was 4,5%. Mean length of stay was 1 day. CONCLUSIONS: the circular stapler devices trough endoanal techniques are feasible, safe and effective with low morbidity. Colorectal surgeons might implement them in the use of them in order to optimize results. Further investigation is required to optimize patient selection and reduce potential complications.

Background: The use of mechanical anastomoses reduce the length of gastrointestinal surgical procedures. Aim: To report the experience with mechanical functional terminal anastomoses in bowel surgery. Material and Methods: Prospective non randomized registry of all mechanical anastomoses carried out in a surgical service. Results: A total of 327 patients (193 women) were analyzed. Two hundred and one anastomoses were ileo-colonic and 126 were entero-enteral. Seventeen percent of patients experienced some complication. Fourteen required a new operation due to peritonitis or deep infection of the surgical site. The global rate of anastomotic dehiscence was 3.1 percent. No differences in the rate of complications between surgeons with experience and trainees were observed. Conclusions: Mechanical anastomoses in bowel surgery are safe and require a short learning period to be used.


Se presenta nuestra experiencia con la anastomosis terminal funcional con sutura mecánica en cirugía intestinal. La serie corresponde a 327 pacientes, un 59 por ciento de género femenino. Del total de anastomosis 201 fueron íleo colónicas y 126 entero enterales. El instrumento de sutura más utilizado fue de 75 u 80 mm de longitud y no se registran incidentes durante la confección de la anastomosis. La morbilidad específica se encuentra en el 17 por ciento de la serie, sin diferencias entre los dos tipos de anastomosis. Catorce paciente fueron reoperados por peritonitis o infección profunda del sitio quirúrgico La tasa global de dehiscencia anastomó-tica fue de un 3,1 por ciento ( 5 y 4 casos respectivamente) sin alcanzar significación estadística. Al comparar si la anastomosis fue realizada por un cirujano con experiencia o en formación, tampoco se encontraron diferencias estadísticamente significativas, por lo que nos parece una técnica segura y con una corta curva de aprendizaje.
